How an Attorney Fills Out Letters of Conservatorship for a Limited Conservatorship of the Person

Attorney Grant A. Toeppen shows how he fills out Letters of Conservatorship for a Limited Conservatorship of the Person for a California Conservatorship.

The Letters of Conservatorship are required by medical institutions, financial institutions, and other entities if you are making decisions for a developmentally disabled individual who is over the age of 18. It is a good idea to have the Letters on file with the Limited Conservatee's primary care physician.

The Letters of Conservatorship are issued by a Court after a Conservatorship has been granted. Certified Letters of Conservatorship are almost always required by the requesting institution. There is typically a $25 cost to get Certified Letters of Conservatorship. You can get a Certified copy at any time after the Limited Conservatorship has been granted.

Nothing in this video creates an attorney-client relationship between me and you, or me and anybody for that matter. Nothing in this video is to be construed as legal advice. It's just how I fill out the form.
