In this video, attorney Grant A. Toeppen shows you how he fills out the form "Duties of Conservator" for a Limited Conservatorship of the Person in the state of California. This form outlines the various fiduciary duties imposed upon the limited conservator once that individual is appointed by the Court.

 

The proposed conservator is required to read this form and certify receipt of a copy of The Conservator's Handbook. This book is available online if you search for it, but some courts hit you up for $20 for a printed copy. It's actually full of really useful information!
